At Lifespan Continu-Care we make it possible for you or your loved one to receive quality care while remaining in the comfort of your own home.   We understand that the requirements of home recovery often go beyond medical services.  So in addition to professional nurses, social workers and therapists, we provide trained home makers, companions, personal care aides and certified nurse assistants.

Services tailored to your needs from just a couple of hours to round-the-clock care.   Whether you are recovering from an illness, are challenged by a lifelong disability, or are simply struggling with the process of aging, Lifespan Continu-Care can start your care on the same day you request service, and can provide services from  as little as two (2) hours  to 24 hours of assistance a day.

At Lifespan Continu-Care, every caregiver is fully insured and has passed an extensive screening process that includes background checks, drug testing and FBI fingerprinting.  Finally, we test each applicant’s knowledge and validate their caregiving skills.

Joint Commission

JCAHO accreditation, now known as “The Joint Commission” accreditation and certification is recognized nationwide as a symbol of quality that reflects an organization’s commitment to meeting certain performance standards. Although widely used in hospital and home health settings, there are few Private Duty home care agencies that either seek, or are awarded Joint Commission accreditation. Continu-Care is proud to be able to display the Joint Commission Gold Seal of Accreditation!

Telephony Timekeeping System

A Telephony Timekeeping System is an important component to providing quality care and accurately calculating your caregiver’s time in your home. When the caregiver arrives at your home, they “punch in” by calling a toll free number to validate their arrival time, and call again to “punch out” the time they are leaving your home.

Same Day Start of Care

Knowing when you are going to need private duty care is often not easy to plan ahead for. For many individuals, the need for support and care in the home can present itself with little notice.  It's good to know that Continu-Care is there for you when you need us!  Whether you need a few hours of care a week, or 24 hour care, Continu-Care can provide you with the assistance you need within the same day you request service. 

Specialized Training in Alzheimer/Traumatic Brain Injury/End-of-Life Care

The care needs of individuals who are struggling with issues associated with Alzheimer’s disease, traumatic brain injury or end of life issues can be unique. Continu-Care provides training to our aides so that they can help these individuals maintain their highest level of independence and enjoy the highest quality of life possible.

RN assessment and supervision of care

An important component to the care provided through Continu-Care is supervision and “oversight” of a registered nurse. Continu-Care nurses have the experience and knowledge to bring added value by being able to help our Clients and their families to adjust to their changing health care needs, and finding their own Total Home Care Solution.

Social Needs Assessment

The Social Needs Assessment is another valuable care component provided by Continu-Care that is provided to determine what areas of support and care OTHER than those that are healthcare related that would help an individual to live a better and more independent life at home and identify potential solutions to those needs.
